Diabetes Care Center
Diabetes is on the rise in the United States, where more than 23 million people have the disease - and a quarter of them do not know it. Diabetes can lead to serious health complications such as heart disease, stroke and even blindness. It's important to know your risks and take control of your health. The Diabetes Care Center at Three Rivers Community Hospital can help.
Get Care
At the Diabetes Care Center, we give you the tools you need to control your diabetes - so it doesn't control you. Knowing your blood sugar levels is just the start. We help you thrive with effective diet, exercise, medication and lifestyle counseling.
To get started with our diabetes education program, you need a doctor's referral. For more information, talk to your primary care doctor or call us at 541-472-7120.
Medical Team
The Diabetes Care Center looks at your diabetes from every angle. Our team of experts includes certified diabetes educators, registered nurses and registered dietitians, and well as a medical social worker and physical therapist. We work closely with your primary care doctor (or obstetrician, for cases of gestational diabetes, or diabetes with pregnancy).
In our group classes, you can connect with others experiencing similar issues. We also offer ongoing monthly support groups, allowing you to stay current with your diabetes care long after your initial meetings with us are complete.
Testimonials
"I came to the Diabetes Care Center to get better control of my diabetes. The personal link that has grown between me and the educators is what I value most. In spite of my poor control, they continue to assist me, to see what I can't feel... that I have to have better glucose control to keep my good health and prevent the complications of diabetes. They work with me to overcome the obstacles that come along. My new insulin pump has given me flexibility that I enjoy. Without the help of the Diabetes Care Center staff, having an insulin pump might not have been possible."
