Before You Arrive
Thank you for choosing Three Rivers Community Hospital. Our staff is
devoted to both clinical excellence and family-centered care that seeks
to meet not only your physical needs but your emotional and spiritual
needs as well.
Please please review the information in our Patient Information section so you are well informed of hospital amenities, the admission and registration process, billing, and your rights as a patient.
Registration
When your physician schedules you for a test, a procedure, or
surgery, the Registration Department is notified that you will be
coming into the hospital. Upon notification the Registration Department
may call you at home to gather pertinent information. If we are unable
to contact you at home or the Registration Department is not notified
of your upcoming visit, you will be asked to complete the registration
process when you arrive. Please check in with the Registration
Department before going to your appointment. If you are coming in for a
nonscheduled service, you will also need to first check in with the
Registration Department.
Insurance
We may collect your portion of your insurance deductible and/or co-pay at the time of registration.
Identification
Photo Identification If you are uninsured, you may need to present this to assist in the registration process.
Insurance Cards To bill your insurance provider, we will need to scan a copy of your insurance card.
Valuables
Because TRCH cannot be responsible for lost personal belongings, we
ask that you do not bring items of great value with you during your
hospital stay. Please keep in mind that items such as eyeglasses,
dentures, and hearing aids require special care. Do not to leave these
items on your meal tray, in drinking cups, or wrapped in a tissue. We
will provide you with a container for their safety. We encourage you to
label your personal items with your name. If you misplace something,
call Guest Services at 541-472-7000 as soon as possible.
Upon Arrival
We are honored to play a vital role in your healthcare. Your understanding of the following information will help us better serve you during this important time.
Parking
Parking for patients and visitors is available in the hospital's east and south parking lots. RV parking is available on request, please call our Security Department at 541-472-7125 to make a reservation or if you need assistance.
Wristband
An identification band is placed on your wrist when you're admitted. Please check that the band has your correct legal name and birth date and let us know if it's not correct. This band ensures that you will receive the correct care. It's important that it remain on your
wrist throughout your hospital stay.
Staff
Each hospital staff member wears a photo identification badge that
includes his or her names and job title. Please feel free to ask staff
members to identify themselves whenever it is helpful to you.
Directory
Inpatient Hospital Directory
Unless you ask us not to, your name will be placed on the hospital
directory patient list when you are admitted. The directory tells us
what room you are in so that calls, flowers, mail, and visitors can be
directed to you as quickly as possible. If you do not want to be on the
hospital directory, please notify your nurse or the Registration
Department at 541-472-7160.
Smoking
Because of Asante's mission as a healthcare organization, Asante is a tobacco-free organization. Smoking and tobacco products are prohibited on all Asante-owned and Asante-occupied properties. This includes all buildings, parking lots, and grounds. This policy applies to Asante employees, patients, visitors, and others doing business on Asante property.
