MyChart is a free, secure, confidential online service, available to Asante patients. Asante patients can now access their personal medical record online, from the privacy of their home or any other location with an internet connection.

Features of Asante’s MyChart include:

• Access to medical records
• After visit summaries following hospital discharge
• Proxy access to your children’s MyChart account
• Online bill pay

How to Sign Up

Access your health records securely,
when you need them.

To sign up for MyChart, you will need an activation code. This code allows you to log on and create your new MyChart user ID and password, which you will then use to log on to MyChart. All patients over age 18 will receive an activation code at the end of each Asante visit, so you can sign up at home when it’s convenient for you.
