Home Monitoring

Home monitoring is telehealth service that uses sophisticated, secure technology to keep your doctor informed of your health condition. Using a touch-screen monitor (like a computer screen) in your home, you will be asked to answer daily questions about your general health and vital signs. This information is sent through telephone lines to your nurse and doctor, who oversee your health status and can even conduct live video visits over the telehealth screen.

Is Telehealth Right For Me?

Telehealth is helpful for people with chronic or high-risk conditions such as:

  • diabetes
  • heart disease
  • wound care
  • recovery and rehabilitation
  • special cases of pregnancy or children’s health
