Diabetes Care Center and Nutrition Services – Grants Pass

Our team evaluates, trains and educates in self-care skills for children and adults with Type 1 and Type 2 diabetes and prediabetes.

Our specialists are trained in diabetes education and behavior change. To meet your needs, our self-management program combines the expertise of your health care provider, certified diabetes educators, registered nurses and dietitians, physical therapists and medical social workers.

Our registered and licensed dietitian nutritionists are trained in medical nutrition therapy and have experience working with patients of all ages with nutrition-related diagnoses. Our dietitians will help develop a plan that works for you and your health while considering your individual needs and cultural food preferences.
