Creekside Family Medicine Joins Asante Physician Partners

Asante is pleased to announce that Creekside Family Medicine in Medford will join Asante Physician Partners effective January 2013.

“We are thrilled to welcome Creekside to our family,” said Sheri Bodager, executive director of Asante Physician Partners, a medical group formed earlier this year that now boasts more than 60 physicians and providers in Jackson and Josephine counties.  “This is another sign that physicians in the Rogue Valley want to work with one another to transform the way healthcare is delivered.”

Asante – the Medford-based health system that includes Asante Physician Partners and its two hospitals: Asante Rogue Regional Medical Center in Medford and Asante Three Rivers Medical Center in Grants Pass – was a logical partner for Creekside. 

“Asante Physician Partners is working with physicians to create a new model of care that improves outcomes and reduces costs,” said Alan Bates, DO, a Creekside physician and state senator who has been a champion for healthcare reform in Oregon and the concept of Coordinated Care Organizations. “The mission here is to improve how healthcare resources such as emergency rooms and admissions to hospitals are utilized. We believe that the best way to accomplish this is by integrating outpatient physicians and teams within the hospitals.” 

Bates said Asante Physician Partners has designed an organizational model that includes physician leadership in the transformation of care.  Physicians are at the table providing input in decisions that impact the healthcare needs of the community as well as day-to-day patient care.

“Asante Physician Partners is working side by side with physicians to improve the coordination of care for their patients,” Bates said. “By joining Asante, we are leading the way to a better healthcare future.”

Creekside Family Medicine will continue to operate with the same providers in its current Medford location at 2859 State Street, Suite 102.

“We are excited to bring Creekside’s physician leadership and more than 30 years of excellent patient care to Asante Physician Partners,” said Roy Vinyard, president and CEO of Asante. “I know they will help us create the right healthcare model for our community.”
