If you're planning for an upcoming trip to Asante Three Rivers Medical Center, please fill out the form below and a staff member will contact you within one business day (Monday-Friday). Filling out the request does not guarantee a room, so a staff member will be in contact with you. You may also request a room by calling us directly at (541) 507-3000 during regular business hours (8 a.m. to 3:30 p.m., Monday-Friday).

• Rooms are $50 per night. If this is a challenge, please call to discuss options.
• The maximum length of stay is 12 consecutive days.
• The maximum capacity per room is three people.
