For many people daily medications are essential to staying healthy. But to be effective, medications must be used safely. When you go to the hospital, you need to know the names of your medications and the names of your doctor or care provider as well as your pharmacist.

By always carrying a list of your prescription and nonprescription medicines — as well as the vitamins and supplements you take and your allergies — you and your care provider are ensuring your safety when you receive medical care in an emergency or are admitted to the hospital.
Fill out the medication list form, keep it with you at all times, and update the information whenever it changes. It’s quick to download, and it makes updating your information a snap!
Complete all the information on the form. List everything that you take. This includes prescription medicines, over-the-counter medicines, herbal medicines and vitamins even if you take them only occasionally.
Be ready
Have your medication list and medical information with you every time you consult a doctor, nurse or pharmacist or go to a hospital or clinic. It could save your life.
Be curious
If there is anything you don’t understand about your medications, ask your doctor, care provider or pharmacist for an explanation.
Be consistent
Use the same pharmacy for all your medications and update your documentation every time there is a change.
Be open
Tell your family about the medications, vitamins and supplements you are taking; when you take them; and where they can find your medication list.
